The doctors and nurses are often very friendly in Japan. They will try to talk to you slowly. If you speak English or a foreign language, they may use this to communicate with you. It may be a good idea to check and transcribe in advance the Japanese that can convey the symptoms and their details.

Feel free to tell your doctor if you have any problems or concerns.

ーExplanation of  required documentsー

Patient registration card

 

The patient registration card is your personal card.  You can get it after writing the patient registration form.  This card will be used every time you go to the hospital. Don't lose it!

You need to fill out the patient registration form only for the first time. It mainly fills in personal information such as your address and phone number. The form varies from hospital to hospital, so if you don't understand it, ask a nearby staff member for help. The above sample has an English translation, but it may be written only in Japanese.

interview sheet

The questionnaire is an interview sheet about your health condition for the day. Your doctor will know your symptoms by looking at this, so write it carefully. This sheet varies from hospital to hospital, so ask the staff if you have any questions. The above sample has an English translation, but it may be written only in Japanese.

prescription

 

A prescription is a document that describes the type and amount of medicine that a doctor needs to treat a patient's illness and how to take it. The pharmacist will prepare the medicine based on it, so take it to the pharmacy.

prescription record

The medicine notebook records information such as the name of the medicine you are using and how to use it, along with the past experience of allergies and side effects.

By presenting it to a doctor or pharmacist at the time of medical examination or dispensing, it is possible to check for drug duplication and swallowing, and to check the history of allergies and side effects.

insurance card

Without a health insurance card, the cost can be several times higher. Don't forget to bring it with you.

residence card

You may be asked to show your residence card, so don't forget to bring it with you.

~At the pharmacy~

There are two types of pharmacies: in-hospital pharmacies and out-of-hospital pharmacies.  Rest assured that most out-of-hospital pharmacies are right next to the hospital.

 Prescriptions are prescribed at the hospital.  Be sure to bring it to the pharmacy as it describes the medicines that will be given.
